Yeah, the middle button on my mouse is very difficult to use to rotate the camera. Often entirely unresponsive. Is there a way to rotate the camera around the pivot point with the keyboard?

You will need to change the Camera Mode in the preferences dialog.

The tkbd's reference guide has a nice chart that shows the controls for each one.
